PROSPECTIVE PUPPY OWNER QUESTIONNAIRE
ALL NEW clients of Shoal Creek Kennel MUST complete this questionnaire before we will respond to any inquiry for a puppy.
The rationale for this stipulation is clearly outlined on our website’s “Prospective Owner Information Page’.
1. How did you hear about Shoal Creek Kennel and why did you choose our Kennel?
2. What types of dog are you interested in and why? (Quiet, lap dog, excitable, size and etc.).
4. Are you interested in a male or female?
3. Are you interested in a puppy, young adult or older dog and why?
5. How and why did you choose__________________ (breed) over other breeds? What specific qualities of this breed are attractive to you? What do you think their weaknesses are? What is your experience with this breed?
6. What kind of experiences are you looking forward to in owning a dog?
7. Have you ever trained a dog? Would you consider taking your dog and attend obedience school with it?
11. Did you ever own a dog before? If so what kind, how long did you have it and what happened to it if you no longer have it?
8. Are you familiar with the grooming requirements for a dog? Would you do the grooming or take it to a shop?
9. Would you have a safe place to walk and exercise your dog?
10. What other pets do you have if any?
12. Do you have a local veterinarian and do you understand the costs associated with taking your pet to one?
13. Have you ever, or would you consider breeding your dog, (even if it is just one litter)?
14. Are you familiar with reasons to/not to spay or neuter you dog?
15. Tell us about your family, children, if any (how many and their age).
18. Describe what you think the typical day in the life of your new dog would be.
17. Who would be the primary care taker of your new puppy?
16. Tell us about where you live, (country, apartment, housing development and etc.).
